Residents of age 65 and over, living in Toronto Community Housing (TCH) building in the Englemount-Lawrence neighbourhood, have a lower attachment rate to primary care services in comparison to other neighbourhoods in Toronto.
This IDEAS team had the following aim statement;
By June 30, 2019, 80% of tenants living at two TCH buildings in the Englemount-Lawrence neighbourhood participating with the north sub-region Interprofessional Primary Care Team (IPCT) will rate their confidence to manage their health condition as a 7 or higher in the Likert scale out of 10.
