At Bramalea CHC, we always look at ways to reduce demand. There is a consistent trend that clients revisit our clinic unnecessarily. We collected data on top 10 episodic conditions and found a revisit rate of 20% for low back pain (LBP) and urinary tract infection (UTI).
Focus groups with clients confirmed their uncertainty about the condition, severity, self-management plan and pain.
Primary care provider education about UTI and LBP Best Practice Guidelines (BPGs) will be implemented using various strategies with the aim of reducing revisit rates for these two conditions.
