At this point in the QIP Program Cycle, teams should be implementing their change ideas and measuring
or monitoring outcomes and impacts of changes. Ontario Health has many
excellent resources to assist teams in the implementation phase, including:
- Plan-Do-Study-Act Tool: Learn how to test and improve your change ideas by following the PDSA
approach. Document your test plan, observations, analysis, and decision to
continue to revise and evaluate your change ideas.
- Ontario Health’s Implementing and Sustaining Changes Guide: Learn about
stakeholder engagement, how to communicate change, standardize change, provide
training, and more.
- Implementation Tool: List how you will support the
standardization, documentation, training, measurement, and resources to
implement the change.
Documenting your work over the coming
months is not only important for learning, but also for your 2027/28 QIP. In
the Progress Report section, you will be asked to reflect on the successes and
challenges you experienced implementing your change ideas over the previous
year. This information is valuable when developing your plan for the coming
year and enables other teams to learn from your experiences.
